<p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p>Name of Product: Tassimo Single-Cup Brewers</p>
<p>Units: About 835,000 in the United States and an additional 900,000 in Canada</p>
<p>Manufacturer: BSH Home Appliances Corp., of Irvine, Calif.</p>
<p>Hazard: The plastic disc, or T Disc, that holds the coffee or tea can burst and spray hot liquid and coffee grounds or tea leaves onto consumers using the brewer and onto bystanders, posing a burn hazard.</p>

<p>Incidents/Injuries: There have been 140 reports of incidents with the brewers spraying hot liquid, coffee grounds or tea leaves onto consumers, including 37 reports of second-degree burn injuries. One incident involved a 10-year-old girl from Minnesota who received second-degree burns to her face and neck and had to be hospitalized.</p>
<p>Description: This recall involves Tassimo brewers with the Bosch brand name and Tassimo Professional brewers. The brewers use plastic T Discs that are filled with coffee or tea to brew hot drinks. “BOSCH” and “TASSIMO” are printed on the front of the brewers. “TASSIMO PROFESSIONAL” is printed on the front of the professional model. The Bosch-brand brewers were sold in black, white, anthracite, gray, silver, red, titanium and white/gray colors. The Tassimo Professional brewers were only sold in black. The following model numbers and date codes are included in this recall. The model number and date code are located on the bottom of the brewer. No other Tassimo brewer is included in this recall.</p>

The Tassimo Professional model was sold directly to hotels and food service providers and they are being contacted directly.</p>

<p>Remedy: Consumers should immediately stop using the recalled Tassimo brewers and contact the firm to order a free replacement T Disc holder for the brewing mechanism.</p>

The T Discs are plastic discs filled with coffee that are inserted into Tassimo coffee makers to brew single cups of hot espresso drinks. They were sold in packages of eight or 16 espresso T Discs. T Discs with codes ending with 11213 through 12020 are included in this recall. The code is printed on the T Disc’s foil lid and on the side of the package.</p>

The DOM (Date of Manufacture) can be found on a white rectangular sticker affixed to the underside of the handle.</p>

An investigation by Pfizer found that some blister packs may contain an inexact count of inert or active ingredient tablets and that the tablets may be out of sequence. The cause was identified and corrected immediately.</p>
<p>These products are oral contraceptives indicated for the prevention of pregnancy in women who elect to use oral contraceptives as a method of contraception. These tablets were manufactured and packaged by Pfizer Inc., commercialized by Akrimax Rx Products and labeled under the Akrimax Pharmaceuticals brand. This product is distributed to warehouses, clinics and retail pharmacies nationwide.</p>

These packaging defects do not pose any immediate health risks. However, consumers exposed to affected packaging should begin using a non-hormonal form of contraception immediately. Patients who have the affected product (lot numbers are provided below) should notify their physician and return the product to the pharmacy.</p>
<p>These products are packaged in blister packs containing 21 tablets of active ingredients and seven tablets of inert ingredients. Correct dosing of this product is important in avoiding the associated risks of an unplanned pregnancy. Correctly packaged blister packs are pictured here.</p>

The product may contain soy protein, an undeclared allergen. People who have an allergy or severe sensitivity to soy protein may run the risk of an allergic reaction if they consume this product.</p>
<p>The voluntary recall was initiated after the Company discovered there may have been a potential cross-contact with product that contained soy protein being produced on the same equipment. There have been no reports of illness as a result of this incident. The Food and Drug Administration has been made aware of this recall.</p>
<p>This recall applies only to the following products: “AUNT JEMIMA FROZEN PANCAKES” packaged in cardboard cartons with the UPC codes listed below, which were sold and distributed nationally, with a ‘Recommended Use By Date’ between November 1, 2011 up to and including October 16, 2012. <p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p>Name of Product: Expandable Insulated Lunch Box with Freezer Gel Pack</p>
<p>Units: About 248,000</p>
<p>Importer: California Innovations Inc., of Toronto, Canada</p>
<p>Hazard: Gel that contains diethylene glycol and ethylene glycol can leak out of damaged freezer gel packs, posing a poisoning hazard if ingested by children or adults.</p>
<p>Incidents/Injuries: The firm has received two separate reports of a dog chewing and ingesting gel from the gel pack. One dog reportedly died from ingestion of the gel. The other was reported to have received treatment and has recovered.</p>
<p>Description: The recalled product is a Ci Sport three-piece, expandable, insulated lunch box set, which includes the lunch box, aluminum bottle and the freezer gel pack. The lunch box of the recalled set has the code &#8220;1-61731-99-57&#8243; printed on one of the two white labels that are sewn under the white fastener inside the main compartment. The lunch box is 8 inches wide, 5 inches deep and 10.25 inches high. It is made of vinyl, polyester nylon and crushed nylon and has a logo with the words &#8220;Ci Sport&#8221; attached to the upper left corner. The lunch box has a carrying handle on the top and a mesh carrying pouch for the aluminum bottle on the side. The aluminum bottle measures 2.75 inches in diameter and 7.5 inches tall. The gel pack is a 6-inch by 4-inch transparent plastic pouch filled with blue liquid gel. The words &#8220;Cryofreeze&#8221; and &#8220;Ice Pack/Hot Pack&#8221; are printed in white letters on the front of the pouch. The lunch box was available in four colors: navy blue, red, black and denim blue.</p>
<p>Sold at: Costco Wholesale Clubs, Leon Korol and Cost U Less stores between May 2007 and September 2008 for about $10.</p>
<p>Manufactured in: China</p>
<p>Remedy: Consumers should immediately stop using the gel packs and dispose of them according to federal, state and/or local regulations. It is recommended that consumers contact their local waste disposal authority for instructions. Consumers may return the lunch box set to Costco for a full refund or may receive a $5 cash refund for the gel pack only by contacting California Innovations.</p>

The axle is a black and made of chromium molybdenum steel with a black steel axle. The front, top of the pedal has “Carbon” printed in red; the top middle has “EPS-R” embossed in the body with the “R” in red and on the top rear of the pedal the word “Forté” printed in white. The outer edge of the pedal has the Forté logos, crossed F’s, in white. The catalog number, 50-8128, is used in all catalogs.</p>

<p>Description: This recall involves Iroda O-Grill models 1000 and 3000 produced before 2010. Some were also sold under the Tailgating Gear brand. Both models are lightweight, portable, clamshell-type propane grills with steel bodies, cast iron cooking surfaces, retractable legs and a handle. They can be used with either 1-pound propane cylinders or 20-pound propane tanks. The grills come in orange, red, green, blue, silver and black and have the words &#8220;O-Grill&#8221; stamped on the metal grill cover. Recalled O-Grills do not have ventilation slots in the regulator cover where the propane bottle screws in. Grills with ventilation slots in the regulator cover are not subject to the recall.</p>
<p>Sold by: LL Bean, Stoneman, BBQG, Walgreens, REI, Dillards and Dick&#8217;s Sporting Goods nationwide from November 2008 through December 2010. The O-Grill 1000 sold for $149 and the O-Grill 3000 sold for $189.</p>

<p>Description: This recall involves the Bugaboo car seat adapter models 80400GC01 and 80401GC02. The adapters are devices designed to attach car seats to stroller frames. They are made of silver aluminum tubing and black plastic connecting parts.</p>

<p>Description: Navien tankless hot water heaters are white with “T-Creator” and “NAVIEN” on the front. A label on the side of the water heater lists the model number along with the manufacturing year in YYYY format.</p>
<p>Sold by: Wholesale distributors to in-home installers nationwide from February 2008 through March 2009 for between $1,500 and $2,100.</p>

<p>Remedy: Consumers should immediately stop using and check the model and manufacture year information on their Navien water heater. Consumers with recalled water heaters should immediately contact Navien to schedule a free repair. Navien will replace all Nylon 66 vent collar with PVC collars. Consumers who continue use of the water heaters while awaiting repair, should have a working carbon monoxide alarm installed outside of sleeping areas in the home.</p>

<p>McNeil Consumer Healthcare Division of McNEIL-PPC, Inc. (McNeil) is voluntarily recalling certain lots of MOTRIN® IB 24 count COATED CAPLETS, MOTRIN® IB 24 count COATED TABLETS and MOTRIN® IB 24+6 count COATED CAPLETS from retailers. The products were distributed in the United States, Puerto Rico, Bahamas, Fiji, Belize, St. Lucia and Jamaica. This is not a consumer level recall, which means that consumers do not need to dispose of or return the product. There is no safety concern if consumers continue taking the product in accordance with its label; however, it is possible there may be a delay in experiencing relief. This action is not being undertaken on the basis of adverse events.</p>
<p>McNeil is recalling these products because testing of product samples showed that some caplets may not dissolve as quickly as intended when nearing their expiration date. Out of an abundance of caution, we are recalling all the listed products since there is a chance they could experience a similar problem as they approach expiration. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p>Name of Product: Circo 17” Children’s Travel Cases</p>
<p>Units: About 139,000 units</p>
<p>Importer: Target Corporation, of Minneapolis, Minn.</p>
<p>Hazard: The surface coating on the travel cases contain excessive levels of lead, violating the federal lead paint standard.</p>
<p>Incidents/Injuries: None reported</p>
<p>Description: The Circo brand label is found on the fabric handle attached to the top of the travel case. The girls’ version has a heart/butterfly/daisy pattern on either a pink or teal background with a plush butterfly attached to the zipper pull. The boys’ version has a pattern of three jet planes in red/blue/green on a red or blue airplane-patterned background with a blue plush jet plane attached to the zipper pull. <p>Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.</p>
<p>O Organics Organic Celery Seed is packaged in a glass bottle with a net weight of 1.8oz (51g) and marked with a UPC 0-7989341124-8. The product was sold at stores from May 25, 2011, through December 16, 2011. Bottles with the following lot codes and “Best By” dates are subject to the recall:</p>

<p>B&amp;M, Inc was notified by their supplier that a lot of Organic Celery Seed had potential Salmonella contamination. The recall is based on a positive result for salmonella that was found during routine testing.</p>
<p>Consumers in possession of this voluntarily recalled product may return it to the store at which it was purchased for a full refund. <p>Swanson Health Products is voluntarily recalling Swanson Organic Celery Seed (Whole) which is packaged in plastic bottles with a net weight of 1.4 oz. (40 grams) because it has the potential to be contaminated with Salmonella.</p>
<p>Salmonella is an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism entering the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.</p>
<p>The product was sold by several different methods. It was either sold at the Swanson Health Products retail store, 109 Broadway N., Fargo, ND, shipped directly to on-line or mail-order consumers, or held for pick up by the consumer at Swanson Health Products headquarters. The product was sold between June 16, 2011, and December 16, 2011.</p>
<p>The lot number of Swanson Health Products Brand Celery Seed (Whole) with code UPC 0-8761424048-0 included in this recall is BESTBY03JUN2013 1541102C. The lot number and Best By date can be found on the bottom of the bottle.</p>
<p>Swanson Health Products became aware of the potential contamination after being notified by its supplier, B &amp; M, Inc., Mount Vernon, Missouri. B &amp; M, Inc. had earlier learned of the potential contamination from its supplier.</p>

<p>Units: About 14,000</p>

<p>Hazard: When the toasters are first plugged into the outlets, the heating element can be energized although the toaster lifter is in the up or off position, which can pose a fire hazard if the toaster is near flammable items.</p>
<p>Incidents/Injuries: Hamilton Beach has received 5 reports of toasters being energized when first plugged into an outlet. There have been no reports of injuries or property damage.</p>
<p>Description: The Hamilton Beach recall involves model 22602 toasters. The model number is printed on the bottom of the toaster. The toaster has a chromed steel exterior, a front control panel with a rotary toast shade selector and function buttons arranged in an arc, a front removable crumb tray and “Hamilton Beach” printed below the control panel.</p>
<p>Sold at: Mass merchandisers and department, grocery and home center stores nationwide from August 2011 through November 2011 for between $19 and $34. Also, some of these toasters were sent to consumers as replacements for model 22600 toasters recalled in June 2011.</p>
<p>Manufactured in: China</p>
<p>Remedy: Consumers should immediately stop using the recalled toasters and contact the firm to receive instructions on how to obtain a free replacement toaster.</p>

<p>As a precautionary measure, certain production lots of Tejava® Premium Iced Tea in 12 oz glass bottles are being recalled nationwide due to the possible presence of glass fragments which could be harmful if swallowed. To date, there have been no reports of any illness or injury.</p>
<p>The 12 oz Tejava® production lot code dates being recalled are:</p>

<p>These production codes can be found on the back of the bottle label and on the long panel of the case tray. They are for case UPC 6-54871-24901-2, four pack UPC 6-54871-04901-1, and single bottle UPC 6-54871-00901-5. Consumers who have purchased the recalled products are asked to return the product to the store where it was purchased and coupons, product replacement, or credit will be issued.</p>
<p>Note: The 1 liter size of Tejava® Premium Iced Tea is NOT affected and is NOT part of this recall.</p>

<p>See’s Candies, Inc. of San Francisco, Calif., is recalling one code of 8 ounce Almond Clusters, because they may contain peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of serious or life-threatening allergic reaction if they consume this product. The total number of boxes produced that may contain peanuts is 3,600.</p>
<p>The product was distributed to See’s Holiday Gift Centers and Licensee locations in Arizona, California, Colorado, Hawaii, Indiana, Kansas, Maryland, Michigan, North Carolina, Nevada, New York, Nebraska, Oklahoma, Oregon, Virginia, Washington, Florida, Georgia, Tennessee, Texas, Mississippi and Louisiana.</p>
<p>The product is sold individually in boxes labeled See’s Candies Almond Clusters, 8 oz. The only code affected by this recall is SF 088/12, and is visible on the back of the box.</p>

<p>Name of Product: Rechargeable External Battery Case</p>
<p>Units: About 6,118</p>
<p>Importer: Mophie LLC, of Paw Paw, Mich.</p>
<p>Hazard: The battery case’s integrated circuit switch can overheat, posing a burn hazard to consumers.</p>
<p>Incidents/Injuries: Mophie has received 110 reports of the product becoming warm to the touch, 44 reports of the product deforming and nine reports of minor burns.</p>
<p>Description: The recalled product is a Mophie Juice Pack Air rechargeable external battery which consists of a lithium polymer battery built into a plastic case designed to snap onto the back of an iPod Touch 4G music player. The battery cases come in black, blue or red. Only battery cases with serial numbers that have the first five alphanumeric characters of TR113 through TR120 are subject to this recall. The serial number can be found inside the housing of the product.</p>
<p>Sold at: B&amp; H Photo, Barnes &amp; Noble, InMotion Entertainment, J&amp;R Music World, Marine Corps Exchange stores, <a href="http://Amazon.com" class="autohyperlink" title="http://Amazon.com">Amazon.com</a> and <a href="http://mophie.com" class="autohyperlink" title="http://mophie.com">mophie.com</a> since April 2011 for about $50.</p>

<p>Hazard: The battery case can overheat while charging, posing a fire hazard.</p>
<p>Incidents/Injuries: CPSC and the firm have received about 14 reports of the battery cases overheating in the United States, including three reports of minor burns to consumers and four reports of minor property damage.</p>
<p>Description: This recall includes the Rocketfish™ Model RF-KL12 Mobile Battery Case. The battery case is made of black lightweight, soft-touch rubberized material designed to hold the phone securely, and comes with a built-in battery. The model number “RF-KL12” appears on the front of the product’s packaging, on the packaging barcode, and in white print on the curved inner surface of the product.</p>
<p>Sold exclusively at: Best Buy stores nationwide, Future Shop and Best Buy stores in Canada, and online from about April 2010 through about September 2011 for between $10 and $60.</p>

<p>To date, the company has not received any reports of consumer complaints relating to this recall. No injuries or adverse effects have been reported regarding these products to date. Ocean Spray issued the voluntary recall out of an abundance of caution to ensure the safety of our consumers.</p>

<p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p>Name of Product: Heath®/Zenith and WirelessCommand® motion sensing wall switches</p>
<p>Units: About 75,000</p>
<p>Manufacturer: HeathCo, LLC of Bowling Green, Ky.</p>
<p>Hazard: When the switches are in the auto mode and the light is off, a small amount of leakage current passes through the electric circuit, including the socket. If consumers fail to disconnect the power at the circuit breaker and make contact with both terminals inside the socket while replacing the bulbs, there is a risk of an electric shock.</p>
<p>Incidents/Injuries: None reported</p>
<p>Description: This recall involves Heath®/Zenith and WirelessCommand® motion sensing wall switches with model numbers listed below. The product replaces a standard household wall switch and is designed to turn off the attached lighting load when motion is no longer detected in the room. The products come in white or ivory. The brand name and model number can be found on a label located on the side of the switch.<br />

<p>Remedy: Consumers should immediately stop using the recalled wall switches and contact the company for a free wall switch replacement.</p>

<p>In cooperation with the Ready Pac Foods recall of bagged salads containing romaine lettuce due to possible E.coli O157:H7 contamination, Safeway is recalling four Safeway Farms brand bagged salads in 14 states. The following salads with the “Best if Used By” dates, Lot Numbers and UPC codes below are subject to the recall:</p>

<p>The “Best if Use-by Date” and Lot Number can be found in the upper left hand corner of the package. The UPC code is located on the back of the bag.</p>
<p>The recalled salads were sold between November 7 and November 16 at Safeway, Carrs, Pavilions, Pak ‘N Save, Randalls, Tom Thumb and Vons stores in the following states:</p>

<p>Ready Pac’s voluntary recall was initiated based on a single positive random sample result for E.coli O157:H7 reported to Ready Pac by the FDA. While no illnesses have been reported to date, customers who purchased this product should discard it or return it for a full refund.</p>
<p>E. coli O157:H7 is a potentially deadly bacterium that can cause bloody diarrhea, dehydration, and in the most severe cases, kidney failure. The very young, seniors and persons with weak immune systems are the most susceptible to foodborne illness.</p>
